For the body of your outline (the portion following the more detailed opening-chapter outlining discussed below), a workable format might be a one- or two-sentence summary of each chapter and an indented paragraph describing its action, with notes about setting and other details given in parentheses. (p. 179 of the Advanced Book Course)

Can someone tell me what does describing its action mean? I'm not understanding what they want.

I believe they are asking you "what happens in the chapter". For instance, your characters sneak into an old museum through a window to look for a famed ghost and are startled by a a manequin wearing an old wedding dress.

Angela described it exactly right. Many writers have chapter summeries full of "she thought...she wondered...she decided...but never tell us what she DID. Be sure to tell us what actually HAPPENS. What are your characters doing?
